What are the 2 ways patients are identified to the breast cancer services?
Through screening or present with lump in breast/axilla.
Brown-green nipple discharge is most commonly associated with which condition?
Duct ectasia
What is duct ectasia?
A dilatation and shortening of the terminal breast ducts within 3cm of the nipple. It is common and the incidence increases with age (post-menopausal). It typically presents with nipple retraction and occasionally creamy nipple discharge.
What is the treatment for duct ectasia?
Patients with troublesome nipple discharge may be treated by microdochectomy (if young) or total duct excision (if older).
The ‘snowstorm’ sign on ultrasound of axillary lymph nodes indicates what?
Extracapsular breast implant rupture.
What is the first-line management for mastitis?
Continue breastfeeding.
A 20-year-old lady presents with a mobile lump in the upper outer aspect of her right breast. On examination she has a firm mobile mass in the upper outer quadrant of her right breast.
Fibroadenoma
A 55-year-old women presents with nipple discharge. On examination she has a slit like retraction of the nipple in the centre of this area is a small amount of cheese like material. No discrete mass lesion is palpable in the underlying breast.
Duct ectasia
A 48-year-old lady presents with discomfort in the right breast. On examination she has a discrete soft fluctuant area in the upper outer quadrant of her right breast. A mammogram is performed and a ‘halo sign’ is seen by the radiologist.
Breast cyst
Why is US the imaging modality of choice in younger women?
Because they have more dense breasts with more glandular tissue, which is easy to image on US scan.
Why should you avoid taking blood or putting a cannula in the arm on the side of previous breast cancer surgery removal?
Because there is a higher risk of complications and infection due to the impaired lymphatic drainage on that side.
What is the most appropriate management for suspected Paget’s disease of the nipple?
Imaging and biopsy
What is the MOA of aromatase inhibitors?
Reducing peripheral oestrogen synthesis.
Graft vs flap
Skin graft receives its blood supply from the recipient site though the vascular bed, whilst a skin flap brings its blood supply from the donor site.
What is the first-line antibiotic for mastitis?
Oral flucloxacillin
