Duct ectasia defintion
Ducts widen and walls thicken. A/w pain, nipple inversion, green/brown d/c
Duct ectasia Mx
Conservative usually suffices (supportive bra, nipple hygiene)
Mammogram/USS to exclude sinister causes
Abx if infected
Duct can be excised if Sx persistent/impactful
Duct ectasia Cx
infection - Abx (abscess needs drain)
Mastitis
Fistulation - duct excision (if wide will leave areola scar)`
Duct ectasia prognosis
Benign disease. NO increased malignancy risk
Severity variable - determines management
Survival not affected
Duct papilloma Mx
Biopsy on lump to determine ?cancer
Surgical excision and complete removal of tumour (bc of possiblity of upgrade to atypical ductal hyperplasia or DCIS on full specimen histology)
Duct papilloma Cx
No significant ones
Post-op: bleeding, infection, pain, fat necrosis, cosmetic deformity
Risk that core biopsy missed some cancer in the papilloma
Duct ectasia Prognosis
90% are without atypia (1.3% invasive cancer, 2.7% DCIS)
Local recurrence is low risk at 2%
