Invasive carcinoma
A
Findings
- Bilateral CC and MLO mammogram.
- Breast density- category 1 ( <25% fibroglandular tissue), category 2 ( 25-50%), category 3 (50-75%) and category 4 (>75%)
- There is an ill defined density within the upper outer quadrant of the breast.
- It is 3cm and is roughly 6cm behind the nipple at the 4 o’clock position.
- There are no axillary lymph nodes identified.
- There are no further breast lesions in the contralateral breast and in my review areas- behind the nipple, retroglandular fat space, in the inferior aspect of the film and in the axilla.
- Further views- coned compression
- COMPARE WITH PREVIOUS STUDIES
- USS
- There is a low echogenic solid lesion.
- It has ill defined thick and irregular borders that disrupt the normal stromal planes.
- It is wider than tall.
- It demonstrates vascularity.
- It demonstrates a posterior acoustic shadow.
- I would USS the axilla to assess if there are any pathological lymph nodes.
- I would ultrasound the contralateral breast to assess if there are any further lesions.
- Intervention-
- 14G biopsy, 4 samples.
- Would not accept a negative biopsy.
Diagnosis
- BIRADS catergoy 5- invasive carcinoma.
- Differential
Management
- Referral to breast surgeons.
- MDT referral.
