discuss breast abcess/ mastitis

A

inflammation +/- infection of breast tissue. – abcess is where pus collects.

commonest cause is milk stasis- inflammation– can lead to infection

staph aureus causes.

pain / redness- wedge distribution.

Rs: if lactating – warm compress, pain relief. continue feeding.

if 12-24 hours after milk removal no improve- oral abx- fluclox 500mg qds. mycin if allergic.
2nd line- co-amoxiclav- 500/125 qds.

48 hrs of abx and no improve/ suspected abcess – general surgeon urgently.

fibroadenoma

A

benign

smooth lumps 1-3cm in size. - considered giant if more than 5cm.

younger women usually 20s-30s.

painless- very mobile under the skin (breast mouse)

Ix: triple assessment - US, biopsy, mam.

Rx: usually nothing
if large or juvinile- excision biopsy, or vacum biopsy.

no increaced risk of breast cancer.

may be oestrogen dependant- enlarge during preg, may hurt before period.
