BPPV Flashcards
common cause of..
recurrent episodes of vertigo triggered by head movement
peripheral cause of vetigo
presentation
variety of head movements can trigger vertigo
common trigger is turning over in bed
symptoms 20-60s for each attack, epidosed occur over several weeks
no hearing loss or tinnitus
pathophysiology
calcium carbonate crystals (otoconia) become displaced into semicircular canals
they disrupt normal flow of endolymph ocnfusing the vestibular system
causes of otoconia to become displaced
no clear cause
viral infection
head trauma
ageeing
diagnostic test
Dix-hallpike manoeuvre
Dix hallpike manouvre
- pt sat up head turned 45º
- rapidly lower head until head hanging off end couch
- hold patients head still, remaining45º
- watch eyes for 30-60s for nystagmus
Dix-hallpike manouvre - what indicates BPPV
rotational nystagmus
symptoms of vertigo
treating BPPV
epley manoeuvre
idea is to put crystals in position that won’t disrupt endolymph flow
