borderline personality disorder Flashcards
tell me what the NICE 2009 guidelines for borderline personality disorder say
- BPD is instability of relationships, self image, mood, and impulsive behaviour. psychotic symptoms, fear of abandonment and rejection, and suicidal thinking or self harm is often present.
- work in partnership to help them be involved in the treatment options and life choices available to them- encourage them to make manageable changes to their life that will help them deal with current problems. develop optimistic and trusting relationship while doing this. be aware transition between services is a big deal for them, so the care plan should help manage this and they should be aware and prepared for any change. meds can’t be used specifically for BPD but can be used for some of the co-morbidities. sedatives can be used in BPD.
- ensure BAME people have access to treatment culturally appropriate to them. ensure any language barrier is aided.
- ensure confidentiality but encourage family to be involved. always take into account their wishes.
- identify manageable short and long (employment) term goals and the steps to achieve them. develop a crisis plan that identifies triggers and self management strategies
- when in crisis: look at their point of view, be empathetic and validating, stimulate reflection about possible solutions to the current problem but don’t offer solutions, offer follow up at a specific time.

what is schiach et al 2021 saying?
as part of DBT, teaching patients with BPD distress tolerance techniques can be useful in helping patients to feel safe and stable to be in situations they previously avoided. it also reduced stress and behaviour problems. more of a long term intervention though as training can be overwhelming. these results have low generalisability as participants had multiple co-morbidities.

what is bailey 2019 saying?
using the SMART criteria for setting goals can be beneficial to actually achieving the goal, however it doesn’t lay out how the goal will be achieved. this is helpful to know while care planning
