Q

Osteosarcoma

A

Malignant prolif of osteoblasts, peak incidence in teenagers, but bimodal with incindence in elderly. Famililial retinoblastoma, paget disease, and radiation exposure are risks. Arises in metaphysis, pathologic fx, bone pain with swelling, “sunburst” and Codman triangle on imaging. Bx pleomorphic cells that produce osteoid

19
Q

epiphysis benign

A

chondroblastoma, giant cell tumor

20
Q

metaphysis benign

A

osteoblatoma, osteochondroma, non-ossifying fibroma, osteoid osteoma, chondromyxoid fibroma, giant cell tumor

21
Q

diaphysis benign

A

enchondroma, fibrous dysplasia

22
Q

sheets of uniform oval mononuc cells and numerous osteoclast type giant cells with 100+nuclei

A

giant cell tumor of bone - benign but locally aggressive. Do not produce bone or cartilage

23
Q

osteochondromas develop only in

A

bones of endochondral origin

24
Q

EXT1 or 2 loss of function mutation

A

Hereditary exostoses - germline mutations (Au Dom)

25
Q

EXT genes encode

A

enzymes that synthesize heparan sulfate glycosaminoglycans. If these are abnormal or absent, diffusion of Ihh may be prevented. Ihh is a local regulator of cartilage growth

26
Q

circumscribed lucencies with central irregular calcifications, a sclerotic rim and an intact cortex

A

chondroma

27
Q

Ollier disease

A

nonhereditary syndrome of multiple enchondromas

28
Q

Maffucci Syndrome

A

nonhered syndrome with multiple enchondromas plus multiple soft tissue hemangiomas. Increased risk of ovarian carcinomas and brain gliomas

29
Q

IDH1 and 2 mutations associated with

A

enchondromas - gene encodes isocitrate dehydrogenase

30
Q

mutated isocitrate dehydrogenase

A

produces 2-hydroxyglutarate - “oncometabolite” that interferes with regulation of DNA methylation

31
Q

well circumscribed nodules of hyaline cartilage containing cytomorphologically benign chondrocytes

A

enchondromas

32
Q

large destructive mixed blastic lytic mass with infiltrative margins

A

osteosarcoma- mc primary malign tumor of bone ~20%

33
Q

RB mutation association

A

osteosarcoma

34
Q

TP53 mut association

A

osteosarcoma

35
Q

A

primary, intramedullary, osteoblastic, high grade occurring in metaphysis of long bones

38
Q

large, gritty gray white tumor with areas of hemorrhage and cystic degeneration

A

chondrosarcoma prognosis determined by

A

tumor grade

43
Q

t(11;22) causes fusion of

A

EWS gene with FL11 gene

44
Q

metastasis to small bones of hand and feet usually from

A

lung, kidney, colon - uncommon

45
Q

produce lytic lesions upon metastasis

A

carcinomas of lung, kidney, GI tract and malignant melanoma
