A
- informed consent
- check what is prescribed
- ensure correct equipment
- verbally check patients information and check with details on bag as well as against wrist band
- check it is the right blood bag
- check the blood bag unit number, expiry, blood group and volume
- monitor patient

- Recognise changes
- Stop infusion immediately
