Skilled Nursing Facility
inpatient healthcare facility with the staff and equipment to provide skilled care, rehab, and other related health services o patients who need nursing care, but don’t require hospitalization
Adult Day Services
designed for people who need supervision during certain hours, but don’t live in a facility where care is provided
Outpatient Care
care, treatment, or procedures that need short-term skilled care that doesn’t require an overnight stay
Acute Care
24-hour skilled care for a short period of time, usually in a hospital
Hospice Care
care given for those who have about six months or less to live
Home Health Care
care or services provided in the home
Assisted Living
facilities for residents who are more independent but may need some help with daily tasks
Long Term Care
24-hour skilled nursing care for those who need a high level of care due to chronic and ongoing conditions

a person receiving home health care
Patient
a person receiving or registered for medical treatment

a patient residing in a long-term health care facility
Medicare

doctor services and other medical services and equipment

Registered Nurse (RN)
licensed professional who has had at least 2-4 years of training and coordinates, manages, and provides skilled nursing care
Certified Nursing Assistant (CNA)
performs delegated tasks such as taking vitals, bathing, eating, etc
Activity Director
plans actives for residents to help them socialize and stay active
Medical Doctor (MD)
diagnoses diseases, and injuries and prescribes treatments and medications
Licensed Practical Nurse (LPN)
licensed professional who has completed at least 1-2 years of training and is able to administer medications/give treatments
Resident/Patient
this person has the right to make decisions about his/her own care
Interdisciplinary
relating to more than one branch of knowledge
Person-Directed Care
care that places the emphasis on the person needing care and their individuality and capabilities
Policy
a course of action that should be taken every time a certain situation occurs
Procedure
a method or particular way you are supposed to do something
