What characterises bipolar disorders?
Episodes of mania and depression
When do bipolar disorders usually begin?
Teens - 20s - 30s
Lifetime prevalence bipolar?
BPI: 0.3-1.5%
BPII: 4% (broader criteria)
Gender spread bipolar?
Equal (M=F)
How are bipolar disorders classified?
- Bipolar I
- Bipolar II
- Bipolar III: depressed pt who develops hypomania during Rx
- Bipolar IV: depressed pt with substance induced hypomania
- Unspecified
What is Bipolar I Disorder?
Presence of at least one full fledged (disrupting normal social and occupational function) manic episode, and usually depressive episodes.
What is Bipolar II Disorder?
Presence of major depressive episodes with at least one hypomanic episode, but no full-fledged manic episodes.
What is unspecified bipolar disorder?
Disorders with clear bipolar features that do not meet the specific criteria for other bipolar disorders
What is cyclothymic disorder?
Prolonged (>2y) periods including both hypomanic and depressive episodes; however episodes do not meet criteria for bipolar disorder.
Aetiology BD?
- Exact cause unknown
- Heredity significant
- Dysregulation of 5HT3 and NA
- Stressful events of a/w onset and exacerbations
Which drugs can trigger exacerbations in patients with bipolar?
- Sympathomimetics (e.g. cocaine, amphetamines)
- EtOH
- Certain antidepressants (e.g. TCAs, MAOIs)
Pattern of BpD?
Acute phase of symptoms followed by repeating course of remission and relapse.
What are BpD relapses?
Discrete episodes of more intense symptoms that are manic, depressive, hypomanic or a mixture. Last from weeks –> 3-6m
Lifetime suicide incidence?
15x that of general population
What is a manic episode?
> 1w of a persistently elevated expansive or irritable mood and persistently increased goal-directed activity or energy plus >3 additional symptoms
What are the additional symptoms characterising a manic episode?
Elevated / irritable mood
- Inflated self esteem or grandiosity
- Decreased need for sleep
- Greater talkativeness
- Flight of ideas
- Distractability
- Inc goal directed activity
- Inc risk taking behaviour (e.g. buying sprees, foolish investments)
- Inc sexual activities
Appearance and manner of manic patient?
May be:
- Exuberant
- Flamboyantly dressed
- Authoritative manner
- Rapid, unstoppable speech
- Clang associations
- Easily distracted (thoughts, projects)
What are clang associations?
New thoughts triggered by word sounds rather than meaning
