What is bilirubin?
By-product of red blood cells breakdown
Why do babies have higher billi production?
Because red blood cells break down faster and have shorter life span.
Newborns produce more than double than adults.
Babies also have slower bili removal through poop.
Unconjugated bili
Indirect
Floating around in the blood by itself
It is fat soluble and can cross blood brain barrier-> kernicterus
It is bound to the protein albumin which delivers it to the liver. The bili that is free can go to the brain.
Conjugated bili
Direct
Inside the liver
Water soluble and can’t go to the brain
If it’s not pooped out it can change again into unconjugated into the body
What is hemolysis?
red blood cells
breaking down in the blood
What is sepsis?
Whole body reaction to infection.
Infection spread in the blood
Physiological jaundice
Normal
60% of term babies. Peak- 5-6 mg/dl
80% of preterm babies
Cephalocaudal progression
From head to the toes
Non physiological jaundice
In the first 24 hours is not normal!
Over production of bili
Or decreased clearance of bili
Bili overproduction causes:
-abo incompebility
-bruising
-caphalohematoma
