Pyometra
Often seen in middle aged and older, undesexed female dogs, usually within three months of being in oestrus. Under the influence of progesterogens, bacterial (usually faecal flora such as E. coli) are able to enter the uterus via the cervix and establish infection if not cleared. Neutrophils and monocytes fight the infection- production of large quantities of purulent exudate in the uterus. “closed” pyometra, a severe inflammatory response ensues and can lead to SIRS (systemic inflammatory response syndrome), sepsis and death. “Open pyometra”- a profuse vaginal dicharge is apparent as the purulent exudate drains through an open cervix into the vagina. Pyometra often seen with UTI.
Why does polyuria and polydipsia occur?
Result of impairment of the kidneys renal concentrating mechanisms. These renal changes are thought to be secondary to immune complex glomerulonephritis and bacterial endotoxin interference with renal tubular response to ADH.
What is the tx or pyometra?
Remove ovaries (source of progesterogens) and the uterus (ovariohysterectomy). Antibiotics on their own are insufficient to resolve the infection.
