what is abennets fracture ?
it is an oblique fracture at the base of the first metcarpal bone which extends to injure the carpometacarpal joint
and it is always associated with subluxation
te distl part ofthe fragmnet iis adducted by the adductor pollicus (ulnar nerve)
the metacarpal shaft displaces proximillay by the abductor pollicus longus ( radial nerve)
what causes bennet fractures
caused by an axial force directed against the partially flexed metacarpal.
when a person punches a hard object
occur as a result of a fall onto the thumb
treatment of bennets fracture ?
closed reduction
and thumb spica splint if minimal displacement
if displaced closed reduction
and percutaneous pinning through through the first metacarpal bone into the trapezium
or from the first metacarpal bone to the second metacarpal bones
more than 3mm - open reduction and internal fixation
also put in a thumb spica splint
what are other first metacarpal fracture ?
orolando fracture - comminuted interarticular fracture through the base of the first metacarpal bone
basal split ino volar and dorsal fragmentos
extrarticular fractures
up to 30 degree angulation tolerated treat with a thumb spica splint
more tan 30 - metacarpal hyperextension
