What is condordance?
an agreement reached after negotiation between a patient and healthcare professional
Respects the believes and wishes of the patient in determining whether when and how medicine are to be taken and the patient
Agenda of the patient is most important
What is compliance
The consistency and accuracy with which someone follows the regime prescribed by a physician
What is adherance?
Adherence is the extent to which the patients actions match the prescribers recommendations
What is unintentional non-adherence?
patient wants to follow the agreed treatment but is prevented from doing so by barriers that are beyond their control
e.g poor recall or difficulties in understanding instructions problems with using the treatment and inability to pay for the treatment or simply forgetting to take it
What is intentional non-adherence
Patient decides not to follow the treatment recommendations
May be due to there believes and preferences not being fully understood when consultations was happening
What is the role of the healthcare professional in prescribing drugs in line with the principle of concordance?
Healthcare professional is there to facilitate and support patient sin there involvement in decision making and support if the decision has been made to prescribe
What are some of the patient information needs?
Need to know what does to take Consequences of taking other drugs how long they need to take the medication for and how Why there are taking the medication what are the chances of improving my condition Are there any side effects What do i do if i miss a does etc etc
What are formal information sources used by patients to find out about medications?
Pharmacists GP's Leaflets given by GP NICE NHS
Informal
Google
Peers
Newspapers and magazines
Why might patients not take medication as prescribed?
Goes against religious beliefs
Patient unable to remember
Patient wants to be sick
Patient has concerns problems about the medication
Patient experiences side effects
What are the requirements for shared decision making?
At least two participants physician and patient
Both parties need to share information
Both parties should take steps to build a consensus about the preferred treatment
An agreement is reached on the treatment to implement
What are the key aspects of a patient centred consultation style?
Effective way of communication that is at the level of understand of the patient
Facilitate patients to make there own decision about the treatment they want to progress with by giving information and advice
be aware of the patients concerns about there medications
Routinely assess adherence in a non-judgemental way
review patient knowledge and there understand and concerns about medication
