What is a regional flap?
Flap of skin from the same area of the body
What is the causative agent of tetanus?
Clostridium tetani
What is a keloid scar?
Scar that extends beyond the original wound
Where are hypertrophic scars more common on the body?
Central chest
Back of shoulders
Ears
What is the difference between a graph and flap?
Graft is transferred from other site and depends on recipient site for vascular supply
Flap is transferred from other site that doesn’t require recipient vascular supply
When is a flap required over a graph?
If the vascular bed is not sufficient
Need to cover a prosthetic material
Better appearnace
What are silver containing dressing for?
Antibacterial
What are vaccum dressings for?
Negative pressurised sponged pushed into wound; sucks out fluid and promotes angiogenesis
Why do you get pressure sore?
Pressure causes local tissue ischaemia
What is a split thickness skin graph?
One that requires recipient blood supply
What is a hypertrophich scar?
Exaggerated normal remodelling response that remains in line with original wound
What secondary intention healing?
Allowing a wound to heal by itself
How do you treat a bite?
Thorough debridement
Antibiotics
Healing by secondary intention - don’t closed it immediately usually
What are hydrogel dressing for?
Maintenance of moist wound
What is the role for prophylactic antibiotics in wound infection?
Not generally used
What is a full thickness skin graph?
Graph including dermis
Limited donor site availability due to requiring suture closuire
What are alginate dressings for?
Absorption of wound exudate
What are some causes of impaired wound healing?
Venous/arterial insufficiency
Immunosuppression/Steroids
Diabetes
Poor closure
Infection
Foreign bodies
Pressure
Smoking
Nutritional
Radiation
Compare spilt and full-thickness skin graphs
Split
- Can cover larger area
- Easier take
- Don’t take adnexal structures
- Poor cosmesis
Full-thickness
- Better cosmesis
- Has adnexal
- Less contraction
- Smaller sites only - limited by donor/recipient site match
What is the different between a local and regional flap?
Local is brought from directly adjacent to the wound
Regional is brought from a close by area eg forearm to wrist
