MOA of Barbs?
Agonist effects at the GABAa receptor
–>At high concentrations directly activate GABA receptor to enhance chloride permeability
Also has allosteric modulations to the GABA receptor
Membrane effects
–>Nonsepcific neuronal depression at high doses
What are the indications for taking a Barb?
Insomnia Seizure management Anxiety Anesthesia neonatal Hyperbilirubinemia
What effects absorption of a Barb?
Its lipid solubility
Can Barbs cross the BBB?
Yes
How are Barbs excreted?
Excreted in Urine as an Anion
What are some AEs of Barbs?
Hangovers
Hyperalgesia –>extremely sensitive to pain
CNS excitation (overly stimulated) –>especially in elderly)
Hypersensitivity (allergic rxns)
Respiratory & cardio effects
–>Hypotension or Hypothermia
–>Respiratory failure***
What are some CI in Barbs?
- Acute intermittent porphyria
- -> leads to demylination
- -> paralysis on CNS - Anyone taking CNS depressants
Are Barbs potent inducers of CYP450?
Yes
What effects do Bicarb have on someone who took Barbiturates?
its increases the excretion of Barbs
Chloral hydrate?
Sedative/Hyptonic
What is Temazepam used for?
Improves sleep maintenance
What are the effects of Benzos on sleep?
Increases the total sleep time
May prevent transition from Lighter stage 2 sleep into deep sleep (Stage 3-4)
