Bacterial Pneumonia 2 Flashcards
1
Q
what are the characteristics of Legionella?
A
- gram - rods
- stains poorly by Gram –> requires silver or IF in tissue sections
- facultative intracellular parasites
- free-living form is motile (flagella) and intracellular form is non-motile
2
Q
what are the 3 outcomes of legionella infections in human?
A
- asymptomatic seroconversion
- pontiac fever
1) flulike
2) incubates hours - 2days
3) resolves w/o complication - Legionnaires Dz
1) pneumonia
2) suppression of kidney function
3) incubates 2-10 days
4) usually resolves with hospitalization & treatment
3
Q
what are the presentation of LD?
A
- altered mental status
- headache
- high fever/chills
- pneumonia/cough/chest pain
- pancreatitis
- diarrhea
4
Q
what is the fast way to diagnose legionella infection?
A
urine antigen test (commercial ELISA kit)
5
Q
treatments for legionella infection?
A
- pontiac fever often resolves w/o treatment
- LD requires an antibiotic that penetrates infected cells
1) levofloxacin
2) azithromycin
3) old school: erythromycin
6
Q
what are the characteristics of mycoplasma bacteriology?
A
- smallest free living organism
- strictly aerobic
- no cell wall
- colonies have “fried-egg” shape
