Impetigo
Skin infection due to S pyogenes and S aureus alone or together.
Cause small thin walled vesicles or pustules that rupture into honey colored crusts.
Tx: Clindamycin or Dicloxicillin
Ecthyma
Superficial infection by Grpup A strep (pyogenes) or S aureus
Bulla/vesicals that errup revealing a healing ulcer with scarring.
Most commonly effect the extremities, no honey crusting!
Treat with Dicloxacillin.
Erysipelas and Cellulitis
When strep gets into lymphatics erysipelas occurs when it involves the deeper dermis and fat.
Cellulitis then occurs.
ERISYPELAS unlike cellulitis has sharply marcated borders that are elevated.
Difference between erysipelas and cellulitis?
erysipelas has well demarcated elevated borders.
Cellulitis is redenning without well marcated borders.
Both caused by strep infections in the deeper dermal lymphatics.
What type of infections are most commonly caused by S. Aureus in the skin?
Abscesses or open wounds
What type of wounds are H. influenza most commonly associated with?
Facial cellulitis in young children.
What type of wounds are P multocida commnly found in?
Cat and dog bites.
What type of wounds are P aeruginosa found in?
Immunocompromised hosts.
Farunce
Infection of the hair follicle.
Erythrasma
Superficial infection usually asymptomatic involving intertriginous areas.
Causitive is Corynebacterium mnutissimum causes Coral Red under the woods lamp!!
Treat with Whitefields ointment or topical miconazole.
Petechiae, splinter hemorrhages, oslers nodes (red painful nodes on pads of fingers and toes) and Janeway lesions (nontender erythematous macules on palms and soles)
Highly suspicious of Bacterial Endocarditis!
Veruca Vulgaris
Most common form of wart on the fingers and hands.
Veruca plantaris
Trauma at pressure sites on the plantar surface of teh foot.
Tender on lateral compression.
Mosaic warts
Very large, diffuse group of warts with ill-defined boundaries.
Best conservative approach to wards?
Acid therapy
60% Salicylic acid
or
80% monochloracetic acid (best for mosaic)
