A
Principal Disorder that needs immediate attention
depression, lapses of suicide thoughts, schizophrenic episodes, etc.

A
Personality disorder that may be shaping or affecting response to Axis I
(PTSD, mental retardation, paranoid personality disorder, etc)

Axis III
A
Medical or neurological problems that may be relevant to the individual’s current or past psychiatric problems
(severe asthma attacks which client could confuse for panic attacks)

Axis IV
A
Identifies major psychosocial stressors
divorce, death of a spouse, move o another city, etc

Axis V
A
Score from 0-100 measuring our highest level of function, 100 being nearly perfect, 0 being an absolute lack of competence.
