C Code 4: Fire or smoke condition from a transformer at any location, i.e., pole, vault, room, etc.
AUC 180
Choose correct for Transformer Vault
A. Greatest hazard is amt of electricity, but asbestos and PCB’s may also be present
B. All transformers in vaults contain dielectric oil w/PCB’s in the 10-49 PPM range. Treat as Non-PCB Transformer and operate as if no PCB’s are present
C. Transformers convert primary voltage (27,000 Volts) to secondary voltage (120 V) and members should only be concerned w/higher voltage
D. Transformer vaults are located underground and easily noticed by members

B FDNY considers a transformer with any amount of PCBs as a PCB Transformer. Operate accordingly.
C Either of these voltages can cause fatal injury if not handled properly.

Operating at Transformer Vault choose correct (3)
A. Due to amt of flammable liquid, stretch a precautionary HL and be prepared to initiate foam ops
B. Teams of 2 shall examine exp for CO readings in meter room, surrounding rooms, and 1st fl
C. Make entry into vault prior to Con Ed if a known life hazard exists
D. Be prepared to re-examine exp on a periodic basis as conditions change rapidly

Smoking manhole and utility co has given 25 minute ETA, choose correct
A. Leave 1 Eng on scene awaiting arrival of Utility Co
B. Protect manhole by traffic cones and tape and place all unit I/S
C. Leave 1 unit on scene to respond to any alarms received; one FF may be left on scene
D. Leave 1 FF at scene and have unit return to qtrs. Off shall provide relief for members
A ETA Under 30 minutes, may leave a unit on scene (preferably an Engine)

Incorrect Con Ed Shift Manager
A. If an incident becomes more severe than originally stated, BC can telephone Con Ed Shift Manager directly, via cell phone
B. If Con Ed ETA is not satisfactory, BC can call Electric Control Center Shift Manager to clarify
C. Response to Con Ed Substation that is not staffed, telephone area’s Electric Control Shift Manager
D. Con Ed has cell phone numbers for each Battalion if they need to speak to on-site BC
C Chief should contact Con Ed Substation Shift Manager via phone
Note: Con Edison Substations do not fall under the jurisdiction of the Electric Control Center

Fire Ice Incorrect
A. Fire Ice coats burning electrical cables, preventing cable fires from extending beyond affected manholes
B. When BC employs Fire Ice- simultaneously request Con Ed Supervisor and Fire Ice Unit
C. Only a Chief can order application of Fire Ice after consulting with on scene Con Ed Supervisor
D. Fire Ice is not effective on Class B fires. Therefore, it is not intended for any type of transformer fire
C When necessary, BC may order application of FireIce prior to the arrival of a Con Ed Supervisor. Objective is to enable BC’s operating at a 10-25 code 1 or code
2 manhole incidents the opportunity to suppress fires prior to the utility company arrival at the scene.

Large padlocks w/hardened shackles given to BC to secure premise when no owner present incorrect
A. Used only on commercial occupancy; these locks shall never be used when fire is encountered
B. FE accomplished w/minimal damage and no other damage to entire perimeter of premise
C. Original padlock hasp and staples in place, and can be easily secured w/padlock
D. Nothing abnormal or unusual about premise or its contents

Incorrect for BC securing premise w/Gravoply lock
A. Battalions carry locks and keys in Battalion car. Keys and lock shall be properly tagged
B. BC shall issue a padlock and key to a company at scene and direct the company to secure the premises
C. 2nd key shall be kept in firehouse HW area that the Battalion is quartered with in case of loss of key carried in car
D. If Unit is at an extended ops when lock is requested to be removed, Dispatcher shall notify BC. BC shall take actions to see lock removed
C Second key shall be kept in Battalion office in event of loss of key carried in car

Correct info for Red Cross at fires
A. Red Cross provides response of their personnel to all 2nd/greater alarms in occupied, unoccupied, or vacant bldgs
B. Red Cross Referral Slip shall only be given to persons needing relocation or assistance as a result of fire as it serves as verification of fire, damage, or residence
C. Persons requiring assistance will make the necessary telephone call to Red Cross per instruction on Referral Slip. Under no circumstances shall FD members make these calls on their behalf
D. Referral Slips need not be given if Red Cross personnel on scene

AUC 231
RAC incorrect
A. At 2nd/greater alarm fires, or as determined by the IC, all FFs exposed to products of combustion shall have their oxyhemoglobin (SpO2), and carboxyhemoglobin (SpCO) levels measured with a RAD – 57 Pulse CO-Oximeter.
B: Members entering rehab should normally rest for approx 20 min before being reassigned. Depending on conditions, the IC, in consultation with the Medical Branch Director, may modify the time frame for rehabilitation.
C: Rehabilitation Group shall be est by the IC at 3rd alarm/greater fires. At long duration and/or physically demanding incidents or in extreme temperatures, the IC should est a Rehabilitation Group.
D: When the Rehabilitation Group is established, the Medical Branch Director will designate an EMS Officer as the Rehabilitation Group Supervisor
C. Rehab Group shall be established by the IC at 2nd alarm or greater fires.

Incorrect for Transformers
A: Con Edison does not test pole mounted transformers for PCB content.
B: Almost all of Con Edison’s overhead transformers are not, or were not, designed to be PCB liquid filled. They are mineral oil transformers.
C: Due to past storage and/or servicing practices some of these transformers have become PCB contaminated, i.e., 50-499 PPM PCB. None will be PCB Transformers (500 PPM or greater).
D: All new transformers being installed by Con Edison as replacements for existing ones and/or new installations are non-PCB transformers.
C Small percentage of these transformers have attained levels of 500 PPM PCB or greater which classifies them as PCB transformers.

Red Cross correct
1) Red Cross referral slips are distributed to all battalions
2) The Red Cross referral slip may be given to anyone who wants one even if the person had no fire damage to their occupancy
3) Box #, date, time of Box, and Division that responded to incident will be inserted on slip when it is given to victims
4) Battalions will no longer use radio communications for relocations of victims of fire
5) The person requiring assistance will make necessary telephone call to the Red Cross per instructions on Referral Slip. An exception to procedure will be made if victim has no access to a telephone or when a severe language problem exists
