A

Autosomal recessive

Triplet repeat in the FXN gene causing a lack of the frataxin protein.

How does Friedreich’s ataxia present?

A
  1. Worsening ataxia
  2. Dysarthria
  3. Distal wasting in the lower limbs
  4. Absent reflexes in lower limbs
  5. Pes cavus (high arch of the foot that does not flatten with weight bearing)

How does Ataxia telangiectasia present?

A
  1. Mild motor delay in infancy
  2. Delay in ocular pursuits and saccades (moving eyes to a target)
  3. Difficulty with balance and coordination, particularly evident at school age
  4. Telangiectasia in the conjunctiva
  5. Subsequent deterioration with complex eye movement disorder

What are the complications of Ataxia telangiectasia?

A
  1. Increased susceptibility to infection and deficiencies in IgA and IgE
  2. Develop malignant disorders (lymphomas and acute leukaemias)
  3. Develop progressive pulmonary disease with bronchiectasis
  4. Have a raised serum AFP
  5. Have sensitivity to ionising radiation

What examination steps do we perform in children with ataxia?

A
  1. Speech - staccato speech (increased separation of syllables)
  2. Neurology - Disturbed proprioception, positive Romberg, nystagmus with eye movement
  3. Systemic - Immunodeficiency in AT, HOCM in Fanconi’s anaemia

A
  1. Cerebral imaging if cause not found
  2. CSF analysis (esp. varicella, strep)
  3. Metabolic screen (inborn errors of metabolism)
