Assessment And Diagnosis Flashcards
Assessment
The process of identifying signs and symptoms of a disease or condition regarding the associated data for purpose of diagnosis
Diagnosis
The process of determining the nature of a disease or condition through evaluation of a patients history, physical exam and resulting lab tests/imaging
Reference standard test
Gold standard to which all diagnostic procedures are compared because it has been shown to be the more accurate
Research studies on newly developed diagnostic test are:
Compared side by side to gold standard
Conducted on patients known to have the disease
Conducted on patients known not to have the disease
Evaluating the validity of a diagnostic test study
Did participating patients present a diagnostic dilemma ( should present NO dilemma)
Did investigators compare the test to an appropriate independent reference standard
We’re those interpreting the tests and reference standard blind to results - should be blinded
Did the investigators perform the same reference standard to all patients regardless of their results under investigation - cannot use different gold standards
Consider for the evidence of accuracy
Postoperative and negative predicative values (PPV/NPV)
Sensitivity
Specificity
Sensitivity
Test is positive when the patient has the disease or condition
Characteristic of test itself
Specific
Test is negative when the patent does not have the disease or condition
True negatives
Characteristic of test itself
Sensitivity
A/a+c
Specificity
D/b+d
Positive predictive value (PPV)
Negative Predictive Value (NPV)
PPV - Percentage of patients with a positive test who actually have the disease
NPV- percentage of patients with a negative test who do not have the disease
Influenced by prevalence, clinical value, not a characteristic of test
Positive Predictive value equation (PPV)
A/a+b
Negative predicative value (NPV) equation
D/c+d
Prevalence
Proportion of individuals in a population having the diseases or characteristic
Disease recurrence adds to prevalence
Reduce prevalence : people die
Incidence
The number of new cases that develop in a given period of time
