Subjective data-
patient feels or perceives
Yellow nails-
pulmonary disease, thyroid disease, fungal infection
White nails-
chronic renal failure, heart disease, diabetes
Emphysema-
respiratory condition where lungs are over expanded with oxygen
Unilateral dilated pupil-
ill nerve compression
Bilateral dilated pupil-
midbrain injury; indication of drug use
Irregular pupils-
orbital trauma
Conjugate gaze deviation-
frontal lobe lesion
Small/pinpoint-
pontine injury, opiate administration
Dilation of eyes affected by-
drugs, oxygen to brain
Levels of consciousness (6)-
-confusion
-Delirium
-lethargy
-obtundation
-stupor
-coma/comatose
Confusion-
confused, disoriented, difficult to reason w/, don’t follow instructions, & may not be able to answer ?
Delirium-
acute, confused state, pt. may be agitated or have hallucinations
Lethargy-
severe drowsiness, often need gentle touch/stimulation to initiate response
Obtundation-
need repeated stimulation to maintain attention & are slow to respond
Stupor-
abnormal drowsiness & only responds to vigorous stimulation
Coma/comatose-
pt. unresponsive to most or all stimuli
2 ways to tell persons awareness-
Grady Coma Scale or Glasgow Coma Scale
Signs of deteriorating LOC-
-lethargy, decreased pulse rate & respiratory rate
-immediately contact Dr. or radiologist
-may have to alter/postpone exam
Hypovolemic-
caused by loss of blood or tissue fluid
Normal respiration rate for adults-
12-20 breaths
Children respiration rate-
20-30 breaths per minute
Newborns respiration rate-
30-60 breaths per minute
normal bp rate for adult-
120/80
pre-hypertension-
120-140/80-89
Stage 1 pre-hypertension-
140-141/90-91
Stage 2 pre-hypertension-
160+/100+
Hypotension-
BP reading less than 95/60
normal resting pulse rate for an adult-
60-100 bpm
normal resting pulse rate for a child-
70-120 bpm
