Grievances: Each grievance form must include:
- Brief statement of grievance and the facts or events on which it is based.
- Sections of contract believed to be violated.
- Remedy or adjustment sought
- Steps taken by grievant to resolve issue
- Maintenance of standards or past practice grievances
- Bargaining unit member’s signature
Notice of delievery of anything in writing required under this article shall be accomplished by confirmed receipt (or Read Receipt) email, to the Chief (or designee), Or Association President.
Step 1: FF must file grievance with an Association Grievance Committee member within ______________ of the date upon which the FF knew of or should have known of the facts or events giving rise to the grievance.

15 CALENDAR days for each of the rest of Step 1 (each step on back of this card)
- Copy of notice of read receipt forwarded to Chief
- Grievance committee determining if a grievance exists.
- If denied by Committee, committee has 15 days to notify grievant.
- Grievant may appeal within 15 days to Executive Board
- Executive Board has 15 to make final detmination.
Step 2: Any grievance found to be valid by the Association shall be submitted to the CHief or the designee within _____________ of the Step 1 ruling.
