Venous Ulcer Mx
- Wound cleaning/dressing
- Compression therapy (To reduce venous HTN)
Venous ulcer Sx
- Occur in gaiter area (lower shin)
- Large/shallow
- Bleed or ooze
- Assoc w/ venous inssuficiency
Arterial ulcer Sx
- Occur distally
- Small/deep
- Clear punch out margin
- DO NOT bleed/ooze
- Assoc w/ weak distal pulses (peripheral arterial disease)
Arterial ulcer conservative Mx
- Wound care
- Lifestyle changes: Be active, Keep hydrated, healthy diet
Arterial ulcer medication Mx
Antiplatelet: Aspirin or clopidogrel
Arterial ulcer surgical Mx
Angioplasty or bypass
Why is compression therapy containdicated in arterial ulcer?
Compression can worsen ischaemia
Aterial/Venous ulcer Ix
- Doppler US
- Ankle-Brachial Pressure Index (ABPI)
Normal ABPI range
0.9-1.30
Low ABPI range
< 0.9
Reduced blood flow due to PAD
Peripheral Arterial Disease
indicate presence of arterial ulcer
High ABPI range
1.3>
non-compressible arteries due to calcification
What is Arterial ulcer?
AKA ischaemic ulcer
open sores that develop due to insufficient blood supply
What is Venous ulcer?
open sore arise from inffucienet venous return from the lower limbs
