blood clot in vessels in heart or brain which reduces blood supply
med emergency= mi/stroke
2 types of stroke
haemorrhagic (blood vessel burst) =high bp, atriovenous malformation aneurysm
2) ischaemic/TIA- mini strokes blood clot blocks brain supply causes :thromboembolism in a fib or atheroscleosis

what determines type of stroke
MRI
ischameic tx
1) antipaltelet i.e clopidogrel 75mg (break clot ) alt dipyramidole MR+ aspirin
tia tx
mr dipyramidole and aspirin (can uses clopidogrel +aspirin but unlicensed in tia )
both TIA/ishaemic strokes
give statin i.e 80mg atrovastation -48hrs after onset of symptoms
- tx HTN target <130/90
intracerebral haemorrhage
avoid aspirin statin and anticoags, treat htn only
if aspirin causing gi problems what can do
co-prescribe ppi
what not to do in ischaemic attack
correct htn, give warfarin
stroke initial management
tia = 300mg aspirin immediate (or alt antipalatelet if not tolerated)+secondary prev
acute ishaemic = alteplase (<4.5hrs of symptoms )+ 300mg aspirin asap within symptom onset (or alt if not tolerated)
