Arterial Line indications
Continuous monitoring of blood pressure- for patients with hemodynamic instability requiring vasopressor medication
Measurement of arterial oxygenation and for frequent blood sampling- used for patients needing frequent blood sampling
Arterial lines contraindications
should not compromise the circulation distal to the placement site
Infection of the site where the catheter is to be placed
traumatic injury proximal to the proposed insertion site
Arterial Line complications
Arterial spasm and the inability to pass the wire or the catheter through the artery are the most common difficulties in catheterization.
What is the normal variation between an auscultated BP and an A-line? Which is higher?
Variation of 5-20 mm Hg is normal
Invasive lines are expected to be higher
What is an Allen Test?
When you compress both the radial and ulnar artery until blood supply is cut off to the hand and the hand turns whiter, then release on of the arteries to test circulation
What are the allen test results?
A normal test is when color is restored to the hand within 5-7 seconds
If it takes 7-15 seconds, filling is impaired
If is takes >15 seconds, circulation is inadequate
What is the proper placement for A-line insertion
1-2 cm from the wrist, between the radius and the flexor carpi radials tendon
What should you do if a patient has an arterial spasm upon insertion
Abandon attempts at catheterization and choose an alternative site
