  1. failure to protect one’s airway
  2. failure to ventilate, charcaterized by a high arterial CO2 level
  3. failure to oxygenate, characterized by low arterial PO2 level

Failure to oxygenate occurs when there is an obstruction of gas exchange between the alveoli and capillaries:

  • possibly due to an interstitial process
  • a defect- or due to a mismatch of ventilation and perfusion (pulmonary embolus)
  • in many lung injuries, it is a mixuture of problems, alveolar dead space, shut and defusion defects
  • patients who are hypoxemic, refractory to oxygen therapy may have right to left intrapulmonary shunting
