Three reasons why someones respiratory appartus would fail
A
- failure to protect one’s airway
- failure to ventilate, charcaterized by a high arterial CO2 level
- failure to oxygenate, characterized by low arterial PO2 level

Failure to oxygenate occurs when there is an obstruction of gas exchange between the alveoli and capillaries:
- possibly due to an interstitial process
- a defect- or due to a mismatch of ventilation and perfusion (pulmonary embolus)
- in many lung injuries, it is a mixuture of problems, alveolar dead space, shut and defusion defects
- patients who are hypoxemic, refractory to oxygen therapy may have right to left intrapulmonary shunting
