List all 7 Initial Documents and describe their purpose. Any order is fine.
- CIS (Contractor Information Sheet) - lists basic information of the contractor in relation to the project. Should include Registration #, CSLB License #, and sub-contractors under them.
- Appendix A (Checklist of Labor Laws) - contractor is aware of the PW requirement and labor codes, including the requirement of CPR/eCPR and/or submission of IWS.
- FBS (Fringe Benefit Statement) - shows where contractors are paying their workers Fringes. Must be signed by each contractor on the project.
- AuthSig (Certification of Understanding & Authorization) - verifies that the person signing the statement of compliance is authorized by the licensed holder (owner).
- PDA (Payroll Deduction Authorization) - indicates a voluntary deduction on payroll that is authorized by an employee.
- DAS 140 (Notice of Award) - notifies an apprenticeship program that a contractor/subcontractor has signed a contract to perform work on a project. Indicates how many JM will be utilized.
- DAS 142 (Request for Dispatch) - requests apprentices under specified craft.
