Lab values indicative of appendicitis
elevated WBC, ANC, & CRP
Diagnostic imaging for appendicitis

Ultrasound
MRI
Common organisms involved in appendicitis
Bacteroides fragilis
Escherichia coli
Pseudomonas aeruginosa
IV hydration
NS or LR
1x maintenance usually used but can commonly e 1.5x maintenance and fluid boluses if required
Analgesia for mild pain
acetaminophen and ibuprofen
Analgesia for moderate/severe pain
Opioids:
- morphine most common
AVOID: codeine and tramadol (CYP2D6 variation)
Antibiotic treatment: Non-perforated appendix
Cefotetan
Cefoxitin
Antibiotic treatment: Perforated appendix
Ampicillin-sulbactam
Piperacillin-tazobactam
Antibiotic treatment duration for appendicitis
average of 5 days
Criteria for discontinuation of IV antibiotics
afebrile
adequate PO intake
ambulating
benign abdominal exam
