Symptoms
Classic Triad: dyspnea, chest pain, syncope
Classic symtoms usually take a stepwise progression fo dyspnea -> chest pain -> syncope
End Stage: Congestive heart Failure
Physical Exam
Harsh late peaking systolic ejection murmur at the Right second intercostal space
Radiating to carotids
Paradoxical splitting of S2
+/- S3, S4
Pulse with low amplitude, slow rise, sustained peak
Brachioradial delay with simultaneous palpation of the rt brachial and radial pulses
Narrowed Pulse Pressure
Complications
Heart Failure
Afib - this is REALLY BAD

Negative inotropics: BB, CCB
Drugs to Use with Caution
Nitrates
Vasodilators
Diuretics
Management of Afib
Cardiovert!!!

Angina: 50% in 5 years
Syncope: 50% in 3 years
CHF: 50% in 2 years
