AOPs Flashcards
Turbine Trip
3550
Entry: Turbine or generator trips
Notes: Anticipate ∆Sk from poisons
Rod withdrawals ≤2 steps/min, dilution ≤15pcm/min
∆P 100-120# for swap to FRBVs
∆P: 60-80# (1-3%), 100-120# (6-9%), 130-150# (10-14%), 160-180# (15-25%)
Verify turb tripped before opening o/p bkr to prevent overspeed
It takes 5-7 sec for exciter field contactors to open
Transition to s/u doesn’t require TG s/d
Rod Control Malfunction
3552 Entry: Uncontrolled rod motion Rods >12 steps misaligned or dropped DRPI or rod motion failure Notes: Urgent failure inhibits all motion for rods on power cabinet
High RCS Activity
3553
Entry: High activity from samples
From Rad Monitor Alarm Response
DC Bus Ground
3551
Entry: Battery Trouble alarm
Ground voltage increasing or >110V
Notes: Checking for ground on chgr, inv, or battery can be in any order
Consult w/mtnc before isolating protective relays or meters
RCS Leak
3555
Entry: Leak
Secondary Chemistry
3557
Entry: MB6 hi conductivity or sodium alarms
Samples out-of-spec
Note: Reducing CWS speed can change seawater intro rate and aid identification of leaking waterbox
Loss of SW
3560
Entry: Partial or total los of SW (pp trip, no press, no flow in SR HXs)
Note: ARPs for pipe rupture
Loss of CCP
3561
Entry: Partial or total loss of CCP (leak, rupture, pp trip, loss of SW)
FO: If RCP bearing oil >195ºF, seal inlet ≥190ºF, seal outlet ≥260ºF, or CCP press can’t be maintained to both CTMT headers → trip the affected RCPs
Loss of One Protective Channel
3564
Entry: Loss of power to a VIAC
TCA: Terminate excess charging ≤8.4 min on CHS malfunction that raises RCS inventory
Note: Loss of VIAC 1/2 deenergizes EGLS
Loss of CTMT Vacuum/Integrity
3565
Entry: Any indication of loss
Emergency Breaking of CTMT Vacuum
3568
Entry: Any condition requiring emergency access
Instrument Failure Response
3571
Entry: Instrument or controller failure that results in abnormal indication or response
Notes: Rods challenge not be left in Auto while diagnosing unless runback
If loop 3 Tc x-tagged or any SF x-tagged, calorimetric shifts to NIs
If calorimetric on NIs, failure of one channel disables it
Rapid Downpower
3575
Entry: Power reduction faster than 0.5%/min (3204) required
Notes: For plant s/d, 30% target is preferred
Rx power to MW correlation varies based on seasonal temps and equipment performance
≤25 min to complete if ISO-NE requested
If SI actuates, rapid boration must be restored
∆T is more accurate power indication than NIs
Adjusting load rate, boration, or rods acceptable for controlling the plant
Maintain 23-25% while using FRVs
Loss of Normal and Offsite Power to 34C/D
3577 Entry: 0V on bus EDG starts and loads 3560/3580/3581, assoc ARPs FO: If RCP bearing oil >195ºF, seal inlet ≥190ºF, seal outlet ≥260ºF, or CCP press can’t be maintained to both CTMT headers → trip the affected RCPs → E-0
Response to Turbine Runback
3579
Entry: Grid instabilities
C-3, C-4, or SWC runback
Turb stop/control valve closure
