benzodiazepine receptor binding making it more prone for GABA binding and opening Cl flow
ADRs of benzos
CNS: ataxia, cog dysfx, depression, dizzy, drowsy, fatigue, irritable, memomry impairment, sedation
Endocrine: decreased libido
GI: appetite changes, wt gain/loss
Resp: nasal congestion
Best benzo for skeletal muscle relaxation
diazepam
Benzos for amnestic effect
triazolam and midazolam (Versed)
Benzo for status epilepticus
Lorazepam
Benzo for petit mal (Lennox-Gastaut variant)
Clonazepam
Dose a benzo for anxiety/ panic disorder
Alprazolam 0.5mg every 8 hours
non-benzo for anxiety
buspirone (BuSpar)
MOA buspirone
serotonin receptor partial agonist
D2 D3 D4 dopamine antagonist
ADRs of buspirone
Rare
CNS: dizzy, confusion, anger/hostility
GI: nausea
Dose buspirone
Buspirone 15mg twice a day
Lithium salts include
Lithium carbonate
Lithium citrate
ADRs of lithium salts
CNS: fine postural hand tremor, electroencephalogram changes
Renal: polyuria & compensatory polydipsia
Endocrine: thyroid enlargement
Cardiac: rare but careful with elderly pts with hx arrhythmias
Do lithium salts need renal adjustments?
Yes and thiazide diuretics and newer NSAIDS reduce clearance
Lithium salts are approved for
Bipolar disorder and mania in bipolar disorder
Dose a lithium salt
Lithium carbonate ER tablet
600mg tablet twice a day
What is the therapeutic range for lithium salts?
04-1.2 mEq/L
If lithium blood level low but s/s improvement dose should be
the same!
ADRs of valproic acid
Wt gain n/v Hair loss easy bruising life-threatening pancreatitis avoid if risk of pregnancy/ younger women (teratogen) suicide risk warning
Valproic acids for bipolar disorder
Depakote, Depakote ER, stavzor
Who can best renally clear valproic acid?
Children <10
What should be monitored when on valproic acid?
Liver function testing monthly to start then every 3-4 months
Dose a valproic acid derivative for bipolar disorder
Depakote ER 500 mg
Three tablets once daily
Anticonvulsants for acute manic episode with bipolar disorder
Carbamazepine/ Equetrol
ADRs of Equetrol
Ataxia, dizzy, somnolence, HA
Heme: agranulocytes, aplastic anemia, leukopenia
Derm: SJS
Suicidal risk warning
Carbamazepine/ Equetrol drug interxns
affect multiple drugs. some rise, some fall
Major CYP3A4 substrate
Strong inducer of CY1A2, CYP2B6, CYP2C19, CYP2C8, CYP2C9, 3A4, and P-glycoprotein
Dose a carbamazepine for bipolar mania
Equetro 400mg twice a day
If severly ill with acute mania
lithium + antipsyc
valproate + antipsyc (not ziprasidone)
