Antivirals - Hepatitis C Flashcards
2 agents for hepatitis C infections
Ribavirin
Interferons
What type of analog is ribavirin?
Purine nucleoside analog
Spectrum of activity of ribavirin
Inhibits replication of a wide range of DNA and RNA viruses
influenza A & B, parainfluenza, RSV, paramyxoviruses, HCV, HIV-1
MOA of ribavirin
(not fully understood)
- interfere with synthesis of guanosine triphosphate
- inhibit capping of viral messenger RNA
- inhibit viral RNA-dependent polymerase
What is the effect of high fat meals on the bioavailability of ribavirin?
Increase bioavailability
What is the effect of antacids on the bioavailability of ribavirin?
Decrease bioavailability
Excretion of ribavirin?
Renal
Clinical uses of ribavirin
- chronic HCV infection
- severe bronchiolitis or pneumonia (children) - aerosol
- influenza virus, parainfluenza, adenovirus, vaccinia, measles - IV/aerosol
Available dosage forms for ribavirin
IV
Aerosol
ADRs of ribavirin
Hemolytic anemia
Depression, fatigue, irritability, rash, insomnia, nausea, pruritis
Contraindications of ribavirin
anemia, ESRD, ischemic vascular disease, PREGNANCY
Why is ribavirin contraindicated in pregnancy?
Because it is teratogenic and embryotoxic
What agents are potent cytokines with antiviral, immunomodulatory, antiproliferative activities?
Interferons
What are the three major classes of interferons?
Alpha, beta, gamma
IFN-a and IFN-b are produced by ___ cells.
ALL CELLS
IFN-gamma production is restricted to ______ and ___ cells.
T-lymphocytes and NK cells
MOA of interferons
- inhibit transcription, translation, post-translational processing, virus maturation, virus release
What is the effect of polyethylene glycol (PEG) on interferons?
- slows absorption
- decrease clearance
- higher and more prolonged serum concentrations
What is PEGASYS?
Peginterferon alfa-2a
What is PEGINTRON?
Peginterferon alfa-2b
Mechanism of clearance of interferon alfa-2A?
via the LIVER
Mechanism of clearance of interferon alfa-2B?
via the KIDNEYS
Dose reduction is required for IFN-alfa 2a and IFN-alfa 2B in what patients?
ESRD patients
ADRs of interferons
- Flu-like syndrome
- transient hepatic enzyme elevations
- mood disorders, depression, somnolence
- MYELOSUPPRESSION
- Fatigue, weight loss, rash
- pneumonitis, cardiotoxicity
Interferons are contraindicated for?
- PREGNANCY
- hepatic decomposition
- autoimmune disease
- cardiac arrythmia
- psychiatric disease, epilepsy
- severe renal insufficiency
- thyroid disease
- ischemic cardiac disease
What is the effect of interferons on theophylline and methadone levels?
Increase theophylline and methadone levels
What ADR is observed when interferons are co-administered with didanosine?
hepatic failure
What ADR is observed when interferons are co-administered with zidovudine?
cytopenia
