How does aspirin work?
- Inhibits COX enzyme
- COX is responsible for prostaglandin and TXA2 synthesis
What are the ADP P2Y12 receptor inhibitors?
- Clopiogrel
- Prasugrel
- Ticagrelor
When should prasugrel dose be reduced?
If weight is less than 60 kg
When is prasugrel contraindicated?
- 75+ yo (unless they have DM or previous MI)
- Previous stroke/TIA
How does dipyridamole work?
Inhibits adenosine and PDE (preventing platelet activation)
Aside from bleeding, what is another ADE of dipyridamole?
Headache
How does cilostazol work?
Inhibits PDE3 (preventing platelet activation)
How do GP IIb/IIIa inhibitors work?
Prevent platelets from crosslinking with fibrinogen
When should eptifibatide be reduced? When is it contraindicated?
- Reduce if CrCl less than 50
- Avoid in ESRD
How does vorapaxar work?
Inhibits PAR-1 to prevent platelet aggregation
How is Vorapaxar given?
ONLY used with aspirin or clopidogrel
When is vorapaxar contraindicated?
Previous stroke/TIA or active bleeding
How do fibrinolytics work?
Convert plasminogen to plasmin to break up fibrin
When is alteplase (tPA) given in MI?
*Only for STEMI-If no PCI performed within 30 minutes
What is the treatment of acute ischemic stroke?
- IV t-PA
- Aspirin 325 mg daily
