AntiHelminths Flashcards
what is the treatment of choice for all schistosome species
praziquantel
katayama fever
systemic hypersensitivity reaction against the migrating parasites which occurs between two to eight weeks after exposure usually treated with corticosteroids or supportive care
albendazole, mebendazole and ivermectin are contraindicated in what pt. population
pregnant patients
DOC for taenia solium and neurocysticercosis
albendazole or praziquantel
DOC for diphyllobothrium latum
niclosamide
DOC for schistosoma
praziquantel
DOC for clonorchis sinensis
praziquantel
only round worm that is not treated with mebendazole
strongyloides stercolis
DOC for strongyloides stercolis
ivermectin
DOC for ascaris lumbricoides, enterobius vermicularis, trichuris trichiura,
mebendazole
DOC for necator americanus
mebendazole
DOC for onchocerca volvulus
ivermectin
DOC for wuchereria bancrofti and brugi malayi
diethlcarbazine and albendazole
DOC for toxocara canis
tiabendazole
DOC for trichenella spiralis
thiabendazole
how is wuchereria bancrofti transmitted
female mosquito (anopheles or culex)
causes filarisis
wuchereria bancrofti and onchocerca volvulus
onchocerca volvulus causes
river blindness
onchocerca volvulus (river blindness) is transmitted by
black fly
DOC for onchocerca volvulus
ivermectin
cestodes are
tapeworms
trematodes are
flukes
nematodes are
roundworms
what are the two tapeworms he wants us to know
taenia solium and diphyllobothrium latum
what are the two flukes he wants us to know
schistosoma and clonorchis sinensis
MOA of benzimidazoles
bind to free beta tubulin and thus inhibit the polyermization of tubulin and microtubule dependent glucose uptake
what is the MOA of albendazole
Selective degeneration of cytoplasmic microtubules in intestinal and tegmental cells of intestinal helminths and larvae; glycogen is depleted, glucose uptake and cholinesterase secretion are impaired, and secretory substances accumulate intracellulary. ATP production decreases causing energy depletion, immobilization, and worm death
what is albendazole used to treat
treatment of neurocysticercosis caused by T. solium and hydatid disease of the liver, lung, and peritoneum caused by echinococcus granulosus
what should you do when you administer albendazole
add an anticonvulsant and steroid therapy during the first week of treatment
what are the adverse reactions for albendazole ?
Dizziness, vertigo, fever, intracranial pressure increase, meningeal signs, and seizures
Mebendazole is used for
treatment of
1) enterobius vermicularis
2) trichuris trichuria
3) ascaris lumbricoides
4) hookworms (ancylostoma duodenale)
what are the contraindications for mebendazole (vermox)
pregnancy (embryotoxic and teratogenic)
steven johnson syndrome
what is the MOA of thiabendazole
inhibits helminth specific mitochondria fumarate reductase
thiabendazole is DOC for
cutaneous larva migrans, trichinosis, and mixed helminthic infections
what is the association of fruit juice and thiabendazole
drinking fruit juice aids in the expulsion of worms by removing the mucous to which the intestinal tapeworm attach themselves
what are the adverse reactions for thiabendazole
angioedema, pruritis, rash, steven’s johnson syndrome
ivermectin is DOC for
onchocerca volvulus and for strongyloidiasis
what is onchocerciasis
river blindness
what is the MOA of ivermectin
glutamate gated chloride ion channels which occur in invertebrate nerve and muscle cells (does NOT cross BBB)
major contraindication of ivermectin
contraindicated in menigitis pts because they have increased permeability and get neurologic side effects such as CNS depression and ataxia
adverse reactions to ivermectin
dizziness, encephalopathy, headache, hyperthermia, insomnia, somnolence, and vertigo
manzzotti reaction occurs with which drug
ivermectin and diethylcarbmazine
what is manzzotti reaction
arthralgia, edema, fever, lyphadenopathy, ocualar damage, pruritus, rash and synovitis
diethycarbamazine is DOC for
lyphatic filariasis (bancrofti and brugia malayi)
pyrantel pamoate MOA
causes the release of acetylcholine and inhibits cholinesterase and acts as a depolarizing neuromuscular blocker, paralyzing the helminths
what is pyrantel pamoate used for
pin worm, whip worm, and round worm
MOA of praziquantel
Increases the cell permeability to calcium in schistosomes, causing strong contractions and paralysis of worm musculature leading to detachment of suckers from the blood vessel walls and to dislodgment
use of praziquantel
schistosomiasis,
adverse reactions in praziquantel
CSF reaction syndrome in patients being treated for neurocysticercosis (HA, seizures, meningimus)`
OA of niclosamide
inhibits mitochondrial anaerobic phosphorylationof ADP in tape worms
what is DOC for tapeworms
niclosamide
hyatid disease caused by
echinococcus granulosus
hook worms species names
N aemricanus
how are hookworms transmitted
direct skin contact with free living soil larvae (walking barefoot)
what is the treatment for hookworms
mebendazole
treatment for ascaris lumbricoides
mebendazole
species name for pin worm
enterobius vermicularis
mode of transmission for pin worm
ingestion of eggs from fingers contaminated by scratching or through contact with contaminated linens or clothening
DOC for pinworm
enterobius vermicularis
DOC for whipworm
mebendazole
visceral larva migrans can cause
granulmatous nodules
what is the treatment for visceral larva migrans and cutaneous larva migrans
thiabendazole
ancylostoma braziliense
dog or cat hookworms that which humans are accidental host
