Antidysrhythmic Medications Flashcards
Vaughn Williams classification:
categorized according to where and how they affect cardiac cells
Class 1 mech of action:
block sodium channels, delay repolrization, used for afib
Procainamide classification:
class 1 ia
procainamide indication:
atrial and ventricular tachydysrhythmias
procainamide adv effects:
SLE like syndrome, torsades de pointes resulting from prolongation of the QT interval
procainamide contraindications:
heart block, SLE
Quinidine indications:
both direction action on the electrical activity of the heart and indirectly anticholinergic effect
Quinidine adv effects:
cardiac asystole and ventricular ectopic beats, tinnitus, GI, vision blurring
Quinidine is what?
BLACK BOX WARNING
Class 1 ib mech of action:
blocks sodium channels, accelerates repolarization, increases or decreases APD
Lidocaine classification:
Class 1 ib
lidocaine indications:
raises the VF threshold
lidocaine adv effects:
twitching, convulsions, confusion, respiratory depression, HYPOtension, bradycardia, dysrhythmias
lidocaine contraindications:
severe SA or AV block, or Stokes Adams or WPW syndrome
what should you watch for with lidocaine?
toxicity
S-slurred speech
A-Altered Cardio Vascular System
M-muscle twitching
S-seizures
Class 1 ic mech of action:
blocks sodium channels, used for Ventricular dysrhythmias
Flecainide indications:
first drug used for a fib, negative inotropic effect and depresses left ventricular function
Flecainide adv effects:
dizziness, visual disturbances, dyspnea
Flecainide contraindications:
cardiogenic shock, second or third-degree AV block and non life-threatening dysrhythmias
Propafenone indications:
mild beta-blocking effects, life threatening ventricular dysrhythmias, a fib
Propafenone adv effects:
dizziness, metallic taste, constipation, HA, N/V
Class 2 beta blockers mech of action:
blocks the sympathetic nervous system, SNS stimulation, and decreases impulse transmission in the conduction system
Atenolol indications:
antidysrhytmic, hypertension, angina
Atenolol contraindications:
severe bradycardia, second or thrid degree heart block, HF, cardiogenic shock
Class 3 mech of action:
increase APD, prolong repolarization in phase 3, used for dysrhythmias that are difficult to treat
