Antidysrhythmia Flashcards
1
Q
Antidysrhythmic Drugs General Considerations
AEs
D-D
contra
A
- All have potential AEs:
> bradycardia
> heart blocks
> arrhythmias
> hypotension
> greater risk w/ IV admin; tele & BP monitor - Drug-Drug: antidysrhythmic drugs, antihypertensives
- Contra: bradycardia, hypotension, heart block
2
Q
Amiodarone - Class & MOA
A
- Class: Potassium Channel Blocker
- MOA: blocks potassium channels, delays repolarization, slows HR
3
Q
Amiodarone - Indications
A
- V-Tachycardia
- V-Fibrillation
- Atrial fibrillation or flutter
4
Q
Amiodarone - Dose
A
- Maintenance: oral
- Acute: IV push/infusion
> tele floors, ICU, ACLS
5
Q
Amiodarone - Drug/Drug Interactions
A
- Incr Digoxin lvls (up to 50-70%)
- Dcr metabolism of Warfarin requiring lower doses (50% incr in INR)
- Dcr doses of either drug by 50%
6
Q
Amiodarone - AEs
A
- Corneal microdeposits
> photophobia, visual halos, dry eyes - Fatigue
- Dizziness
- Photosensitivity
- Thyroid dysfunction
7
Q
Amiodarone - Black Box
A
- Hepatotoxicity
- Pulmonary toxicity
- Pro arrhythmias (new/existing)
8
Q
Amiodarone - Nursing Considerations
A
- Teach: no grapefruit juice
- Use barrier sun block
- Tele monitoring (IV)
9
Q
Atrial Fibrillation Treatment
A
- Goal HR: <110bpm
- Antidysrhythmia + Anticoagulant
> amiodarone, metoprolol, diltiazem
> warfarin, rivaroxaban
