Anticoagulants General MOA
Prevents clot extension and formation (does not dissolve clots) by interrupting coagulation pathways.
Anticoagulants Indications
Prevents DVT after surgery or with prolonged bed rest. Clotting disorder?? - look this up
Anticoagulants Contraindications
Coagulation disorders, ulcer disease, malignancy, recent surgery, active bleeding
Anticoagulants Assessment
Signs of bleeding or hemorhage,unusual bruising. Evidence of additional or increasing thrombosis
Anticoagulants Lab Tests
INR, PTT, PT (Insert values that we are looking for)
Anticoagulants Antidotes
Heparin - Protamine Sulfate
Warfarin - Vitamin K
Anticoagulants Pt Education
- Avoid activities that can lead to injury
- soft bristle toothbrush, electric razor
- report unusual bleeding or bruising
- ## don’t take OTC meds unless you talk to health care provider first
Heparin MOA
Heparin inhibits conversion of prothrombin to thrombin by inhibiting Xa. This stops conversion of fibrinogen to fibrin.
Warfarin MOA
Interferes with hepatic synthesis of Vitamin K dependent clotting factors (II,VII,IX,X).
