Anticoagulant Quiz Flashcards
Match the correct responses:
Warfarin
Heparin
Reverses warfarin
This clotting factor has the shortest half life
1) Normalized prothrombin time is used to measure the pharmacologic effect
2) Phytonadione
3) Factor VII
4) The normalized aPTT is used to measure the pharmacologic effect
Warfarin – 1
Heparin – 4
Clotting factor – 3
Reverses warfarin – 2
Which statement about warfarin is correct?
A) Foods containing large amounts of vitamin K can antagonize warfarin
B) Warfarin exerts its effect on coagulation through activation of antithrombin III
C) Patients should stop eating fruits and vegetables when treated with warfarin
D) Warfarin has an immediate effect on coagulation
A
Match the medication with its mechanism of action:
Dabigatran, heparin, warfarin, rivaroxaban
1) _______ activates antithrombin
2) _______ is a vitamin K antagonist
3) _______ is a direct thrombin inhibitor
4) _______ is a factor Xa inhibitor
1) Heparin
2) Warfarin
3) Rivaroxaban
4) Dabigatran
Your patient today is a 70 y/o M with afib, started on warfarin and well managed for the past 7 months. He presented to the ED were severe bleeding per rectum. Labs: PT 29 and INR 4.9. PTTT is 34. Hemoglobin is 10 g/dL and his platelets are 212,000.
what is your best option to reverse warfarin in this acute and critical situation?
A) FFP
B) Vitamin K
C) Protamine
D) Prothrombin complex concentrate
D – when patients present with an elevated INR following treatment with warfarin, the classic antidote is vitamin K. Vitamin K can be given IV or IM as a 10 mg dose and will start working in about 12 to 24 hours.
Patients who are bleeding acutely with an elevated INR usually need four-factor prothrombin complex concentrate.
FFP lacks sufficient coagulation factors 5 and 8. FFP should not be used as to expand fluid volume.
Protamine is used to reverse heparin, not warfarin.
