Antibiotics (cell wall inhibitors) Flashcards
Penicillin-G (natural)
Works on Gram+ Strept, anaerobes
B-lactamase sensitive
1st tier
What specific bacteria is penicillin G effective against?
Pasteurella, Arcanobacterium, Listeria monocytogenes
Spirochetes: leptospira and borrelia
Ampicillin, amoxicilin (semi-synthetic aminopenicillines)
Gram +, anaerobes and Gram - (E. coli, Klebs, proteus)
B-lactamase sensitive
1st tier
Clavamox
Amoxicillin used to treat infections in all tissues except (CNS)
Anti staphyloccal penicillins
Meth-, naf-, oxa-, cloxa-, dicloxacillin
Active against penicillinase-producing staph spp.
MRSA (S. aureus), MRSP (S. pseudointerdeius)
Extended-spectrum penicillins
3rd and 4th tier
Addition of virulent Gram -
Pseudomonas, serratia, enterobacter
Examples of extended spectrum penicillins
Carbapenems: Meropenem
Carben-, ticar-, mezlo-, piperacillin
Which extended spectrum penicillin is a public health concer?
Carbapenem- resistant enterobacteriaceae (CRE)
PK of penicillins
Poor lipid solubility (weak acid)
Poor penetration of BBB, inside cells, in eye, mammary gland and prostate
Cross placental barrier (found in milk)
Penicillin delivery
Per Os (not for strong infections)
IM and IV with sodium and potassium salts
IM only for less solubles forms
Penicillins are considered _________ dependent drugs
Time
T > MIC 50%
Q4-6h dosing
Renal excretion
Active tubular excretion, concentrated in urine
Probenecid used in combo with penicillins to delay renal excretion
Na+, K+ salt soluble IV formulations
Short duration of action (4hrs)
Procaine penicillin
P-G added with procaine salt
Only IM (or SC) 24 hrs duration of action
Benzathine Penicillin
Benzathine salt with P-G
Only IM (or SC) long acting (3 days)
