Mild community acquired pneumonia.
Amoxicillin.
If penicillin allergy: doxycycline.
Severe community acquired pneumonia.
Co-amoxiclav IV + Doxycycline/Clarithromycin IV.
If penicillin allergy:
Levofloxacin IV
Severe hospital acquired pneumonia or aspiration pneumonia.
IV amoxicillin + metronidazole + gentamicin
If penicillin allergy:
IV Co-trimoxazole + metronidazole +/-gentamicin.
Non-severe hospital acquired pneumonia or aspiration pneumonia.
PO amoxicillin + metronidazole
If penicillin allergy:
Co-trimoxazole + metronidazole.
Acute exacerbation of COPD.
- amoxicillin.
2. Doxycycline.
Sub-acute endocarditis of native valve.
amoxicillin + gentamicin
Acute endocarditis of native valve.
Flucloxacillin.
Endocarditis of prosthetic valve or suspected MRA
Vancomycin IV + Rifampicin + Gentamicin.
Rifampicin should be added in delayed, on day 3-5.
Severe endocarditis of native valve + risk of resistant pathogens.
Vancomycin IV + meropenem IV.
Severe C. Diff infection
Vancomycin +/- IV metronidazole
Non-severe C. Diff infection.
Metronidazole
Peritonitis/biliary tract/intra-abdominal infection.
IV amoxicillin (streptococci and enterococci) + metronidazole (anaerobes) + gentamicin (aerobic coliforms).
If penicillin allergy:
Vancomycin IV + metronidazole + gentamicin.
Mild proven spontaneous bacterial peritonitis.
Co-trimoxazole
Severe proven spontaneous bacterial peritonitis.
Piperacillin/tazobactam.
IV drug user endocarditis.
Flucloxacillin.
