Antibiotics Flashcards
1
Q
Vancomycin
A
- Action: during cell-wall synthesis it binds to D-alanyl-D-alanine and inhibits PG cross-linking
- Activity: cidal
- Spectrum: narrow
Mostly G+ cocci and bacilli (esp. penicilinase-resistant S. aureus) - Pregnancy safe
- Treatment of choice for MRSA
2
Q
Penicillins
A
- Action: during last stage of cell wall synthesis it blocks transpeptidase and inhibits PG cross-linking
- Activity: cidal
- Spectrum: narrow->broader
- Pregnancy safe
- B-lactam rings cleaved by bacteria with B-lactamase (penicillinase). 10% risk of hypersensitivity.
3
Q
Cephalosporins
A
- Action: transpeptidase block
- Activity: Vidal
- Spectrum: narrow->broader
- Pregnancy safe
- B-lactam rings, 10% cross-hypersensitivity with penicillins
4
Q
Metronidazole
A
- Action: inhibits DNA synthesis
- Activity: cidal
- Spectrum: narrow (anaerobes and some protozoa)
- NOT pregnancy safe
- Pseudomembranous colitis, disulfiram-like reaction with alcohol
5
Q
Fluoroquinolones
A
- Action: inhibit DNA gyrase (topoisomerase)
- Activity: cidal
- Spectrum: broader (G+, G-, Mycobacteria, NOT anaerobes)
- NOT pregnancy safe
- Disulfiram-like reaction with alcohol
- Ex: Ciprofloxacin, Ofloxacin
6
Q
Aminoglycosides
A
- Action: binds 30S -> block formation of initiation complex -> inhibit protein synthesis
- Activity: cidal, G+ and G- aerobes ONLY
- Spectrum: broader
- NOT pregnancy safe
- Ototoxicity, nephrotoxicity, teratogens
- Ex.: Streptomycin, Gentamycin
7
Q
Macrolides
A
- Action: binds 50S -> block tRNA release -> inhibit protein synthesis
- Activity: static
- Spectrum: narrow
- Pregnancy safe
- GI upset
- Ex.: Erythromycin, Clarithromycin, Azithromycin
8
Q
Clindamycin
A
- Action: binds 50S -> block tRNA release -> inhibit protein synthesis
- Activity: static
- Spectrum: narrow
- Pregnancy safe
- Ass. with diarrhea and pseudomembranous colitis (overgrowth of C. difficile)
9
Q
Chloramphenicol
A
- Action: blocks 50S -> blocks peptidyl transferase -> inhibits protein synthesis
- Activity: static
- Spectrum: broad
- NOT pregnancy safe
- Bone marrow toxicity, gray baby syndrome
10
Q
Tetracyclines
A
- Action: binds 30S -> block aminoacyl-tRNA binding -> inhibit protein synthesis
- Activity: static
- Spectrum: broad
- NOT pregnancy safe
- Divalent and trivalent cations inhibit absorption, staining teeth during calcification
- Ex.: Tetracycline, Doxycycline, Minocycline
11
Q
Sulfonamides
A
- Action: competes with PABA -> inhibits folic acid synthesis
- Activity: static
- Spectrum: broad
- NOT pregnancy safe
- Ass. with hypersensitivity, renal toxicity, hematopoietic toxicity
- Ex.: Sulfadiazine, Sulfamethoxazole, Trimethoprim
12
Q
Neomycin
A
- topical
- aminoglycoside
13
Q
Polymyxin B
A
- topical
- alters cell membrane permeability
14
Q
Bacitracin
A
- topical
- inhibits cell wall synthesis
