Thyroid hormones consists of…?
Triiodothyronine T3 and Tetraiodothyronine T4 /thyroxine/ . These hormones contains 59-65% iodine and calcitonin.
Adult daily iodine dose
150mcg /200 mcg pregnancy and lactation and up to 250mcg for children /
Thyroxine is absorbed
Best in the duodenum and ileum
Lower doses of warfarin required in
Hyperthyroidism and higher doses in hypothyroidism
Anti thyroid agents
- Thioamides /methimazole, propylthiouracil PTU /
- Iodides / lugol solution and potassium iodide /
- Adrenoceptor blocking agents /propranolol /
Propranolol treat for
Hyperthyroidism, thyroid storm, tachycardia, hypertension, atrial fibrillation /duration 4-6 hours, orally /
Propranolol toxicity
Bronchospasm, AV blockade, hypotension, bradycardia
Thyroid preparations
- Levothyroxine T4 /half life 7 days, daily dose 75mcg /
- Liothyronine T3 /half life 1 days, daily dose 25 mcg /
Higher thyroxine adverse effects
Atrophic gastritis, H pylori gastritis, celiac disease, lactose intolerance
Thyroxine take time
Empty stomach , 30-60 minutes before meals, 4 hours after meals, or bedtime
What is the Myxedema Coma?
End stage of the untreated hypothyroidism. Levothyroxine intravenously 300-400mcg initially.

Euthyrox, levoxyl, novothyrox, tirosint, unithroid
