Vasodilators types?
- Organic Nitrates
- Nitroprusside
- Alpha Blockers
- Sildenafil
- Hydralazine
- Minoxidil
Organic Nitrates drugs?
- Eg. GTN
* Oral, Sublingual, Transdermal
Organic Nitrates mechanism of action?
• NO2- release, ↑ NO release, ↑ cGMP activity, ↓ MLCK activity, ↓ MLC phosphorylated
= leading to vasodilation
Organic Nitrates Side Effects & Contraindications?
- Do not use with sildenafil, excessive NO activity
- Reflex tachycardia (consider giving β blockers)
- Headache, Hypotension, Syncope
- Tachyphylaxis due to decreased GSH levels
Nitroprusside drugs?
• IV administration
Nitroprusside mechanism of action?
• Broken down to release NO, ↑ cGMP activity, ↓ MLCK activity, ↓ MLC phosphorylated
= leading to vasodilation
ALMOST SAME AS NITRATES
Nitroprusside Side Effects & Contraindications?
May be broken down to release CN and is therefore toxic
Alpha Blockers drugs?
• Prazosin
- Can be used for BPH with Hypertension, as urinary sphincter under Alpha-1 control
Alpha Blockers mechanism of action?
• Block alpha receptors leading to vascular dilation
Alpha Blockers Side Effects & Contraindications?
• Headache and dizziness
Sildenafil drugs USED FOR?
• Used for ED and pulmonary hypertension
Sildenafil mechanism of action?
• ↑ NO formation, ↑ cGMP activity by inhibiting PDE5, ↑ MLCK inactivation leading to vasodilation
Sildenafil Side Effects & Contraindications?
• Do not use with GTN for excessive NO activity
Hydralazine drugs?
• Often combined with GTN
Hydralazine mechanism of action?
• ↑ cGMP, ↑ MLCK inactivation causing vasodilation
