Name examples of ACEi. Describe MOA.
RAMIPRIL, LISINOPRIL
Inhibit ACE in lungs, preventing conversion of angiotensin I to angiotensin II. Results in vasodilation, decreased aldosterone production and decreased SNS activation.
Suggest ADRs of ACEi.
- dry cough - increased bradykinin
- hyperkalaemia
- angioedema
- AKI - prevent vasoconstriction of efferent arteriole by AngII (auto-regulatory mechanism to maintain GFR)
Name 3 indications for ACEi
- HTN
- diabetic nephropathy
- secondary prevention of IHD
Name possible contra-indications for ACEi
- pregnancy and breast-feeding
- renal artery stenosis
- aortic stenosis
- hereditary angio-oedema
- hyper-sensitivity
what must be monitored when a pt takes ACEi?

Name examples of CCB. What is MOA?
AMLODIPINE, NIFEDIPINE
Inhibit calcium transport across myocardial and vascular smooth muscle cells… vasodilation of peripheral, coronary and pulmonary arteries.
Give examples of CCB ADRs.
- peripheral oedema
- headaches
- dizziness
- flushing
- tachycardia and palpitations
- gingival hyperplasia (rare)
Name examples of thiazide-like diuretics. What is MOA?
INDAPAMIDE, CHLORTALIDONE
Inhibit Na/Cl symporter in DCT… decreased Na and H2O reabsorption.
At lower doses, vasodilatory effect is prominentl; at higher doses, diuretic effect becomes more apparent.
Suggest ADRs for thiazide-like diuretics.
- postural hypotension
- hyponatraemia, hypokalaemia and hypercalaemia
- hyperuricaemia and gout
- impotence
- impaired glucose tolerance
- hypochloraemic alkalosis
Rare
- pancreatitis
- photosensitive rash
- agranulocytosis, aplastic anaemia, leucopenia
