Anti-Helminths Flashcards
** What is albendazole?
- treats hookworms with a SINGLE DOSE (esp. for mass treating programs in developing countries), which are the leading cause of iron deficiency anemia in developing countries.
- treats toxocariasis and cystercercosis
** How does Benzimidazole (BZA) work?
thiabendazole, mebendazole, albendazole…
- inhibits microtubule polymerization by binding to B-tubulin, thus cuasing degenerative changes in integumental and intestinal cells of helminths, resulting in immobilization and death of parasite.
- selective and irreversible inhibition of glucose uptake by the parasite.
- for SOIL TRANSMITTED NEMATODES (STN)
What drug do you use to treat strongyloidiasis?
Thiabendazole (chewable tablet after a meal)
*** Does albendazole ore mebendazole have better absorption?
ALBENDAZOLE, so use mebendazole for GI dwelling nematodes.
What is Pyrantel pamoate?
- anti-helminth drug used for ascariasis and enterobiasis
What does Piperazine do?
paralyzes ascaris by acting as an agonist at GABA receptors. IT is antagonistic to pyrantel pamoate.
** What is Ivermectin? (TEST QUESTION)
- derived from Strep avermitilis (soil sample from a Japanese golf course).
- Given as a single dose on EMPTY STOMACH (highly lipophilic) to clear microfilaria from the skin.
- used for onchocerciasis (RIVER BLINDNESS), STRONGYLOIDIASIS, lymphatic filariasis, SCABIES, and cutaneous larva migrans.
- for mass deworming also
What is a hydatid cyst?
Echinococcosis= tapeworm infection that affects the liver, lungs, brain, and other organs.
Anytime you see gross hematuria what should you think?
Schistosomiasis= flatworm that can cause portal HTN and hematuria
** What is praziquantel? (TEST QUESTION)
treats schistosomiasis and for mass deworming!
Are there any vaccines for parasitic infections?
NO
Remember, what will be elevated with parasitic (helminthic) infections?
IgE and eosinophil counts
What is a common symptom of Paragonimiasis?
hemoptysis
*treat with praziquantel
*** What is Trypanosoma cruzi? (protozoa)
the kissing bug transmitts this protozoa causing Chaga’s disease, which can cause dilated cardiomyopathy, megacolon, and megaesophagus (dysphagia or odonyphagia)
*diagnose via serology
What are some signs of Chaga’s disease?
- Romana sign (signs at portal of entry)
- fever, systemic symptoms
- hepatosplenomegaly
- heart block
