Anti-Depressents Flashcards
What SSRI has the highest risk of CYP450 drug-drug interactions?
Fluoxetine (potent CYP450 inhibitor)
What are the symptoms of serotonin syndrome?
Sweating, hyperreflexia, akathisia/myoclonus, shivering/tremors
Which SSRI’s are least likely to cause drug-drug interactions?
Citalopram and sertraline
What is the MOA difference in secondary vs. tertiary amine SNRI’s?
Tertiary- Inhibit NE/5HT equally
Secondary- Inhibit NE>5HT
What class of drugs are the TCA’s in?
SNRI’s
What 3 receptors do TCA’s act on to produce key side effects?
Alpha adrenergic (CV effects), Muscarinic (anti-cholinergic), and H1 (CNS sx)
What symptoms would a toxic TCA overdose cause?
3 C’s (“Tri C”)
Coma, Cardiotoxicity (cause of death), Convulsions
What is the MOA of SARA’s?
Block serotonin reuptake presynaptically, and a1 (Trazadone, Nefazodone) or a2 (mirtazapine) postsynaptically
What is the MOA of Bupropion?
NRDI- Inhibits serotonin, NE (via NET), and dopamine (via DAT) reuptake
What MOAI is selective for MAO-B?
Selegiline is B-selective
What is the biggest limiting factor risk of MAOI’s?
Risk of hypertensive crisis with tyramine ingestion (except Selegiline)
What mood stabilizing agent reduces the risk of suicidal ideations?
Lithium
What drug classes interact with lithium to cause decreased renal clearance?
Diuretics, ACEI’s, and NSAIDS
Clinical nephrogenic DI can be caused by what drug?
Lithium
What anti-seizure meds are also used as mood stabilizers?
Valproate, Lamotrigine, and Carbamazepine
