Define intrapartum?
Time between onset of labor and birth
Define postpartum
Time from birth until woman’s body returns to pre-pregnant state, which is about six weeks
Define gestation
Number of weeks since the FIRST day of your last menstrual period
Define abortion
Birth that occurs before 20 weeks
Spontaneous - miscarriage
Therapeutic - medically induced
Define preterm
Birth that occurs after 20 weeks, but before 37 weeks
Define stillbirth
Fetus born dead after 20 weeks
What is term pregnancy?
Pregnancy that is at least 37.0 weeks
What is early term?
37 weeks through 38 weeks and six days
What is full-term pregnancy?
39 weeks through 40 weeks and six days
What is late term pregnancy
41 weeks through 41 weeks and six days
What is post term pregnancy?
42 weeks and beyond
Define gravida
Total number of pregnancies, regardless of duration/outcome, INCLUDING present pregnancy
Define nulligravida
Woman who has never been pregnant
Define primigravida
Woman who is pregnant for the first time
Define multigravida
Woman who is in second or subsequent pregnancy
Define para
Birth > 20 weeks, regardless of outcome
Define nullipara
Woman who has NOT given birth at > 20 weeks… like a miscarriage
A patient is first time pregnant with twins, how would you code that?
G1P1
Twins or triplets etc, count as one pregnancy, and one delivery
What is term (T) in TPAL?
Number of infants born at >37.0 weeks, regardless of outcome
What is preterm (P) in TPAL?
Number of infants born between 20-36.6, regardless of outcome
What is abortion (A) mean in TPAL?
Number of pregnancies ending in therapeutic or spontaneous abortion (< 20 weeks)
What is living (L) mean in TPAL
Number of currently living children
What is Nageles rule?
It determines someone’s due date
How to use Nageles rule
Begins with 1st day of LMP
Ex. July 30th
Subtract 3 months
Ex. July 30th - 3 months = April 20th
Add 7 days
Ex. April 20th + 7 = April 27th
What is quickening?
Fetal movements felt by mother
May indicate fetus is nearing 20 weeks
Can be felt as early as as 16 or as late as 22 weeks
Multiparous women usually report quickening before primigravidas do
How you asses fetal heart beat
Ultrasonic Doppler is the primary tool for assessing FHT’s
May detect at 10 to 12 week gestation
Normal FHT is 110-160 bpm
What is a good prenatal schedule in a healthy pregnancy?
Every four weeks until 28 weeks
Every two weeks until 36 weeks
After 36 weeks, weekly until delivery
What do you give mom if she is RH negative
She’ll need a shot of Rogam.. and if the baby is RH + after delivery.. she’ll need another shot of rogam
What do we test for in the third trimester?
Screening for Group B Streptococcus (GBS) at 36-37.6 weeks
GC/CT vaginal swab (gonorrhea, and chlamydia)
Danger signs in pregnancy?
Sudden gush of fluid from vagina
Vaginal bleeding
Abdominal pain
Temp > 101
Dizziness, blurred/Double vision
Persistent nausea, vomiting, severe headache
Edema to face, hands, feet, legs
Absence or decreased fetal movement
What are some physiological maladaption in pregnancy?
Increased anxiety or depression
Inability to communicate
Inappropriate responses/actions
Inability to cope with stress
Indications of substance abuse
Denying pregnancy
Intense preoccupation with sex of baby
Failure to plan/prepare for baby
Why do we screen for GBS?
Because if you are positive and we don’t treat it.. the baby is at higher risk for infection like sepsis or pneumonia.. that could kill the baby
