Antenatal Testing Flashcards
Non stress test
Measures baseline FHR, accelerations, and variability
Reactive test = 2 accelerations of 15 beats/min above baseline lasting 15 seconds each in a 20 minute pd
Usually done after 32 weeks
Contraction stress test
Must have 3 contractions lasting 40-60 seconds in 10 minute pd
Approximates stress a fetus might have in first stage of labor
Usually done when no danger of preterm labor or uterine rupture
Negative CST
No late decels with adequate contractions
Positive CST
Late decels with > 50% of contractions
Suspicious CST
Inconsistent late decels < 50%
Unsatisfactory CST
Inadequate uterine activity
Biophysical profile
Utilizes real time US and an NST
Measures - NST, fetal breathing movements, fetal tone, fetal movement, amniotic fluid vol.
Max score of 10 (score of 0 or 2 for each category)
Total must be an even #
Fetal breathing movements in BPP
At least one episode of > 30 seconds in a 30 min pd
Gross body movement in BPP
3 discreet body, limb movements in 30 minutes
Fetal tone in BPP
One episode of active extension w/return to flexion of fetal limbs, hands, or trunk
NST in BPP
Reactive
Amniotic fluid volume in BPP
At least one pocket of amniotic fluid vol measuring 2 cm in two perpendicular planes
Interpreting BPP
10 points - Normal Repeat weekly.
8 - normal repeat weekly or twice weekly in diabetics or postdates. Oligohydramnios = deliver.
6 - Suspect Chronic Asphyxia if greater than 36 weeks deliver.
4 - Suspect Chronic Asphyxia if > or = to 32 weeks = deliver.
0-2 - Strongly suspect asphyxia= deliver regardless of age.
When to test
Diabetes - 28 -40 weeks depending on type. Preeclampsia - at diagnosis >26 weeks. Post Dates - 41-42 weeks. IUGR - at diagnosis. Previous Stillbirth - at 32 weeks.
